Phase II Report (310 CMR 40.0835)
A Phase II Report is prepared to document information obtained as a result of Comprehensive Site Assessment activities and support conclusions and Opinions based upon the findings of the assessment. The Phase II Report references or incorporates elements of the Phase I Report and may be combined with the Phase III Report.
A Phase II Report:
presents, contains, or appends relevant information, data, findings, and Opinions related to the Comprehensive Site Assessment of the disposal site;
sets forth in narrative and in maps, graphs, and tables, the approach, methods and results of the Phase II - Comprehensive Site Assessment including:
- Disposal Site Name, Location and Locus Map,
- Detailed Disposal Site Map(s),
- Disposal Site History,
- Site Hydrogeological Characteristics (details of subsurface investigations conducted at the disposal site, a comprehensive description and depiction of site hydrogeologic conditions, including:
- Environmental Fate and Transport of Oil and/or Hazardous Material,
- Nature and Extent of Contamination,
- Exposure Assessment,
- Risk Characterization, and
- Conclusions (a summary of findings and the reasoning and results used to support the findings).
Possible Outcomes of a Phase II Report are the determination:
- for the necessity of Comprehensive Remedial Actions at the site to achieve a Response Action Outcome and a Phase III study for the identification, evaluation and selection of Comprehensive Remedial Action Alternatives is initiated; or
- that the requirements of a Class A or Class B Response Action Outcome have been met and a Response Action Outcome Statement supported by information provided in the Phase II report is submitted to the Department.
